Kinesiology, Nutrition, & Chiropractic are the topic of today's blog. Chiropractors sometimes use an art known as kinesiology to determine what nutrional need a person has. Kinesiology can also be used as a way to diagnose vertebral subluxations, which are the specific problem that chiropractors seek to correct. Some physicians believe that a nutritional problem is also cause of subluxations. Kinesiology is essentially muscle testing. It takes practice to master, and even with a lot of practice, some doctors never seem to acquire the "touch" necessary to provide this service successfully. To do a kinesiologic exam, the doctor first tests a strong muscle, for example having the patient hold up his arm and then pushing gently with steady pressure. It is not a battle of strength. Then exposing the patient to an irritant that the patient's body rejects should cause the patient to be unable to hold the arm up with the doctor's steady push. Irritants can represent allergens, such as cat hair; or if the personhas an issue with their liver, merely placing a hand over the individual's liver is enough to weken the arm. Then introducing a nutrient that will potentially correct the problem, such as a liver detox herb, can then prevent the arm from weakening from the same push, with the hand over the liver. Using kinesiology to diagnose subluxations is done in a similar fashion. If the person has neck pain, the chiropractor can touch specific vertebra while pushing on the person's arm. WHen the problem vertebra is touched, the arm goes weak, and hence the doctor now knows exactly where to make the correction.

Many of my patients come to me for help with a specific problem. These problems range from headaches, neck pain, back pain, tingling and numbness, shoulder pain, knee pain, hip pain, rib pain, and for injuries sustained in an automobile accident. I do my best to teach people about how chiropractic helps, and the importance of continuing their chiropractic caqre once they are out of pain. First of all, many problems are not fully corrected the minute that the pain and symptoms go away. Even after they have reached maximum improvement, there are benefits to coming to your chiropractor on a maintenance basis.

Do you bring your car in for an oil change every 3000 miles? Do you bring your car in for other maintenance on a regular basis? You should, if you want to avoid bigger problems with your car and if you want your car to perform at maximal efficiency. Your body wants to perform at maximal efficiency as well. Eating a well balanced diet low in fat and carbohydrates, getting proper rest and exercise, having emotional and spiritual balance in your life, and having no nerve interference are all important factors to consider when striving for a quality life with health problems. There are other factors as well, such as taking care of you teeth, not smoking or putting harmful substances into your body, and the list goes on. Taking care of your spine is probably not high on the list - but it should be on the top. Think about it for a moment. Your spine is an extension of your brain, with your nerves branching off of the spine to extend control over every function in your body. Even your blood vessels and glands that control hormone balance are controled by nerves. Having a joint out of its normal position and plane of motion can irritate the nerves that are associated with that joint. All nerves have an intimate relationship with a joint, especially the vertebral joint in your spinal column. When joints are out of alignment and nerves become irritated you may feel pain, stiffness, and discomfort. But you may feel fine also. The effect could instead be affecting your organs and your health.

How could nerves affect your health? Remember, everything is controlled by nerves. Cut the all the nerves to an organ and it dies. Irritate the nerves to an organ and soon disease starts to take hold. We live dynamic active lives and have many opportunities to insult our joints, causing them to become subluxated, or thrown out of there normal positions and motion. And so, now you know why it is important to see your chiropractor on a regular basis, to prevent this from happening. Catch the problem before it attacks your body with disease or pain. I recommend seeing your chiropractor once per month if you are feeling fine. When you have a health problem, see them three times per week! I also recomend that you include visits to other types of health professionals as well, such as your acupuncturist, massage therapist, nutritionist, and medical doctor. Never listen to advice that tells you NOT to see your chiropractor. That would be nonsense! Chiropractic is the most natural form of healing. Would you rather have your body put back into proper joint position (where it belongs) so that there is no nerve irritation and so that your health can express itself naturally? Or would you rather just take a drug to cover up your pain, leaving the cause of the problem there to cause further damage. Or would you rather get surgery? The choice is yours! I would rather go the most natural and least invasive route first! I hope that you will choose this for yourself as well.

Carpal Tunnel Syndrome

Carpal Tunnel Syndrome is experienced as pain to the wrists. But is the problem in the wrists? The answer is: sometimes yes and sometimes no. Usually carpal tunnel is instigated by overuse of the wrists that create an inflamation to the sheath of ligament that covers the nerves of the wrist, called the carpal tunnel. However, this problem can also be caused by irritation to the nerves and nerve roots at any point from where they exit the spine, to the shoulder, elbow, and wrist. When a doctor can't figure out what is causing a patient's condition or why they have such a wide variety of symptoms, sometimes they conclude that you must be a hypochondriac and imagining all these symptoms. Attempts to correct the condition may then cease.This method of dismissing a patient's complaints makes the patient feel a sense of self-doubt and insecurity. If attempts to improve your health are unsuccessful, then correction of the misalignments in the skull and spine may be the missing link in your treatment program. The brain and spinal cord - the two major components of the central nervous system (CNS) - require a carefully controlled physiological environment in order to develop and function efficiently and effectively. The craniosacral system is largely responsible for providing this environment.
The craniosacral system is a physiological system that meets the criteria for classification as semi-closed and hydraulic. It has a watertight boundary largely provided by the external layer of the meninges (the membranes that surround the brain and spinal cord). This external layer is known as the dura mater or dural membrane. The craniosacral system's controlled fluid inflow is provided mainly by the choroid plexuses, with controlled outflow provided largely by the arachnoid granulation system. The fluid within the craniosacral system is cerebrospinal fluid (CSF), which is extracted from blood by the choroid plexuses and returned to blood by the arachnoid granulation system. The extraction and resorption of CSF are accomplished largely through osmotic pressures and specialized active transport mechanisms.
The functions of the CSF are carried out as it circulates within the craniosacral system. It circulates between the cells of the brain and spinal cord, and fills the spaces between cells. CSF also crosses cell membranes to enter intracellular compartments, though entry is selective. Some components of CSF are excluded. CSF functions include:
1. Floating the brain so the effects of gravity are reduced by about 75-80 percent;
2. Serving as a shock absorber so the brain is not traumatized by striking the inner surface of the cranium during sudden movements of the head or body;
3. Providing nutrients to the brain and spinal cord, as well the pituitary and pineal glands, both of which are enclosed in the craniosacral system;
4. "Washing away" metabolic waste products and toxic substances that have been deposited within and between cells of the CNS;
5. Lubricating between cells of the CNS to prevent friction or improper contact that could short-circuit electrical nerve impulses or damage cell walls; and
6. Maintaining the proper concentrations of electrolyte substances (ions) necessary for creating and transmitting nerve impulses so we can think, remember, feel and move.
The dural membranes of the craniosacral system form the lining for several cranial (skull) bones. These membranes also attach to specific areas of bone in the spinal canal. The spinal canal attachments are much more sparse than the intracranial attachments, which allows for movement of the spine. When dural membrane movements are abnormally restricted for any reason, the craniosacral system may become compromised, with secondary effects on the CNS and/or endocrine and immune systems.
The volume of CSF in the craniosacral system constantly and rhythmically rises and falls at about six to 12 cycles per minute. This ongoing volumetric change requires dural membrane boundaries to continually adapt to avoid excessive fluid pressures on the delicate brain and spinal cord structures. Part of this accommodation is accomplished by the cranial bones opening and closing minutely where they abut each other.
It is now known that under normal circumstances cranial bones do not fuse together at puberty, as was previously taught.1-6 Rather, they are in constant motion to accommodate the changing demands placed on them by the dural membrane as it adjusts to the rhythmical rise and fall of CSF volume.
Disease, dysfunction or injuries may cause loss of bone mobility in the head, spine or pelvis. Such trauma can cause craniosacral system dysfunction, with secondary ramifications in the CNS, endocrine and immune systems.
The Role of Cranial Adjusting
Cranial Manipulation focuses on:
1. Impaired mobility of all bones related to the craniosacral system (cranium, face, vertebral column and pelvis). Sutures (joints between cranial bones) can frequently become immobilized due to traumatic jamming, surgical procedures, cranial fractures or other injuries. All craniosacral system-related bones are vulnerable to abnormal stresses and tensions imposed by attached muscles, tendons, ligaments and fasciae.
These bones may also be motion-restricted due to the immobility of bones to which they are joined by joints, etc. For example, a leg can compromise the pelvis, which in turn can affect the sacrum to which the dural membrane is attached. The result is a strain and probable functional compromise of the craniosacral system.
The therapist locates these contributing factors and restores motion wherever possible. Thus, the therapist works with total body mobility. When full mobilization is not possible, the therapist helps the client's body develop the best compensatory mechanisms, with the goal of compromising craniosacral system function as little as possible.
2. Impaired accommodative abilities of the dural membrane system. It is imperative that the dural membranes move freely in the vertebral canal, and are able to expand in the cranium. Restrictive factors include bone restrictions and abnormal tension patterns within the dural membrane itself. These are often broadcast into the cranium from ligamentous and/or muscle attachments that are excessively tight due to adhesions or cohesions formed subsequent to infections, surgical interventions, bleeding and/or toxic deposits.
3. The flow of cerebrospinal fluid and blood. The free flow of both is estimated and enhanced as much as possible by the therapist. The membranes of the craniosacral system separate blood from CSF, but it is imperative that blood flows into the choroid plexus and through the arachnoid membrane system in order for CSF to be formed, reabsorbed, and able to do its job. Many cranial techniques focus on enhancing CNS blood supply and venous drainage. There are also many techniques that aim to accomplish the best possible CSF circulation. They are all generally modified for optimal application to each individual client.
4. Body-mind integration. Since the craniosacral system has such influence on the physiological environment of the brain and the two aforementioned glands, many integrative processes between the psyche and the body emerge.
The advanced therapist is trained to assist this integrative process through the use of the hands, as well as by verbally encouraging images that may begin to surface in the client's conscious awareness during a session. The therapist may encourage the client to understand the meaning of the images to get in touch with his or her bodily functions and how he or she may be able to favorably influence them.
Cranial adjusting, just like chiropractic spinal adjusting, primarily facilitates processes that enhance the body's innate abilities for natural healing. Cranial therapy focuses on removing restrictive forces and obstacles related to the craniosacral system. Enhancing the mobility of the craniosacral system improves the circulation of both blood and CSF to the brain, spinal cord, pituitary gland, pineal gland and the cranial nerve systems, among other things.
Improving fluid motion and exchange specifically enhances the functions of the brain; spinal cord; autonomic control systems; visual, auditory, olfactory and gustatory sensory systems; motor and motor coordination systems; endocrine system; and the immune system. Less directly, it seems to affect all other body systems; therefore, it is exceptionally useful for most chronic conditions and as a preventive measure.

Chiropractic Effectiveness

Chiropractic Effectiveness

Chiropractic is now firmly established as a primary health-care profession where, according to the American Chiropractic Association, approximately 7 to 16 percent of people in the United Stated seek treatment each year. Our profession has earned recognition for its remarkable effectiveness and its focus on natural, drug-free, non-invasive methods of treatment. This section summarizes the effectiveness of chiropractic for some of the conditions we treat.

General Lower-Back Pain
Approximately 25% of our patients come to us with a chief complaint of low-back/pelvic pain or discomfort. This pain can be brought on by an accident, overuse, repetitive stress, emotional stress, or just everyday living.

Several studies have been conducted to determine the efficacy of spinal manipulation as a treatment for lower-back pain in general. One of these involved three chiropractic colleges in California, where researchers reported that "the trend for spinal manipulation to produce better results than any form of treatment to which it was compared was consistent and strong." For 86% of the outcomes, spinal manipulation was more effective than any other treatment rendered for lower-back pain.

Additional studies conducted both in and out of the United States yielded similar results, where patients reportedly recovered at a more rapid rate, had fewer relapses, and gained a new sense of confidence in their movement. Still more investigations have been conducted on the effectiveness of chiropractic treatment for two sub-categories of lower-back pain: acute and chronic.

Acute Lower-Back Pain
A report published in 1994 by the Agency for Health Care Policy and Research on acute low-back pain concluded that spinal adjustment is one of the most effective treatments for this condition. In another study published in 1994 by the University of Pittsburgh Medical Center and the Jefferson County (Missouri) Rehabilitation Center, it was concluded that greater and more rapid improvement occurred when treated using exercises and spinal adjustment for acute low-back pain.

A third study conducted by the Los Angeles College of Chiropractic and the University of Vermont was published in 1992. These researchers reported that spinal adjustment, when compared to other methods of treatment for acute lower-back pain (massage, corset, and TMS), proved to be a "superior," more effective treatment after three weeks of care.

Chronic Lower-Back Pain
In 1997, the Dutch Health Insurance Board funded an investigation that summarized the results of 48 separate studies on the effectiveness of chiropractic treatment for chronic lower-back pain. The investigation determined that spinal adjustment is more effective than treatments suggested by general practitioners (bed rest, analgesics, and massage).

Two similar studies conducted in Canada in 1985 (University Hospital in Saskatoon) and 1991 (University of Calgary), plus a third study conducted in Poland in 1986 (Silesian Medical School), produced results similar to the Dutch investigation.

Neck Pain
Approximately 19% of our patients come to us with complaints of neck pain or discomfort. This pain can be caused by a variety of factors, such as an accident, injury, overuse, etc. The most common neck condition we address is whiplash.

The efficacy of chiropractic for neck pain has been well documented in numerous studies conducted in the United States, Canada, Wales, and the Netherlands. In a joint U.S./Canadian study published in 1996, researchers from UCLA, the West LA Veterans Affairs Medical Center, and two chiropractic colleges found that for some patients with sub-acute or chronic neck pain, spinal adjustment proved to be a more effective treatment than mobilization or physical therapy.

In a study conducted by the Canadian Memorial Chiropractic College in Toronto in 1997, researchers found spinal adjustments to improve neck mobility and decrease neck pain. An additional Canadian investigation published in 1997 by the University of Calgary found spinal adjustments to improve neck mobility and decrease neck pain in patients having back and/or neck complaints.

General practitioners in private practice at the Welsh National School of Medicine conducted a study of the efficacy of spinal adjustment in alleviating neck and shoulder pain. The results, published in 1983, indicated that pain and discomfort improved significantly after adjustment, as well as range of motion and flexibility of the affected areas.

A 1992 Netherlands study conducted by doctors of medicine and other professionals investigated different methods of treatment for patients with chronic back and neck pain. After 12 weeks of care, manual therapy (which consisted of adjustment and mobilization of the spine) had showed a "faster and larger improvement in physical functioning" relative to the other types of treatment such as massage, exercises, and physical therapy.

Head Pain
Headaches, both tension and migraine, are the chief complaint for approximately 13% of our patients. Before you reach for your pain-reliever of choice the next time a headache or migraine sneaks up on you, consider the following information on the efficacy of chiropractic care for head pain.

In a 1995 study conducted in Minnesota, the effectiveness of spinal adjustment relative to the administration of an antidepressant drug (amitriptyline) for patients with chronic tension-type headaches was investigated. The researchers included doctors of chiropractic, a doctor of medicine, and another professionals from Northwestern College of Chiropractic and the Pain Assessment and Rehabilitation Center, Ltd. They determined that, after four weeks of treatment, patients receiving spinal adjustment therapy experienced a continued lack of symptoms, whereas patients taking the medication returned to original, baseline conditions.

Another study, published in 1997 by doctors of medicine and doctors of chiropractic at the University of Odense and the Nordic Institute of Chiropractic and Clinical Biomechanics in Denmark, compared spinal adjustment and soft tissue therapy for the treatment of benign, chronic headaches. They found that both methods of therapy allowed for significant improvement, and there were no side effects or signs of worsening associated with either method.

Carpal Tunnel Syndrome (CTS)
With a large percentage of the population using keyboards either at work or at home, CTS is becoming increasingly common. CTS is a condition where there is a disturbance of nerve function in the wrist. Because it is a neurological issue, we are treating more and more patients of all ages suffering from this malady.

One study, published in 1998 and conducted by researchers from the Northwestern College of Chiropractic in Minnesota, compared chiropractic treatment of CTS to conservative medical treatment. The chiropractic treatment included manipulation, ultrasound treatment, and wrist supports, while the medical group received an anti-inflammatory drug (ibuprofen) and wore wrist supports. They found that both methods of treatment proved to be equally effective and recommended that patients with CTS who are sensitive to medical side effects pursue chiropractic for treatment.

Fibromyalgia
Fibromyalgia is a syndrome characterized by chronic pain in the muscles and soft tissues surrounding joints, fatigue, and tenderness at specific sites in the body. While the cause is unknown, an estimated 3 million people are affected in the United States. Because we offer a conservative, hands-on approach to health care, some patients suffering from fibromyalgia are supplementing their medical care with chiropractic care.

In 1997, a study was conducted by three Canadian chiropractors on the effectiveness of chiropractic care for patients with fibromyalgia. They reported significant improvement in flexibility and reduction of pain levels and recommended that this type of treatment be included along with medical treatments for this condition.

Infantile Colic
Colic is a condition that occurs in early infancy and is characterized by episodes of loud crying, apparent abdominal pain, and irritability. Unfortunately, it is a common condition that affects approximately 10% of babies in the United States and lasts from about 2-3 weeks of age until 3-4 months. While the causes of colic are not always known or understood, parents of children exhibiting symptoms of colic are seeking help from chiropractors because of its effectiveness in reducing the severity of the symptoms.

One study that documents the efficacy of chiropractic for treating infantile colic was conducted in 1989 by a doctor of medicine and doctors of chiropractic at the Anglo-European College of Chiropractic in Great Britain. Researchers reported that 94% of infants with colic appeared to be helped by spinal manipulation within 14 days of the start of treatment. The babies tended to have both fewer episodes of crying and shorter crying spells.

Another study, this one conducted in 1999 in Denmark, investigated two infant groups: one that was treated with spinal manipulation, and another that was treated with an over-the-counter antacid medication (dimethicone). After two weeks, the group treated with spinal manipulation exhibited a 67% drop in daily hours of crying, while the dimethicone group experienced a 38% drop.

Just a sampling
In summary, the above-referenced studies indicate chiropractic is effective for head, neck, and back pain, as well as carpal tunnel syndrome, fibromyalgia, and infantile colic. It is important to note that the conditions discussed in this section are not the only conditions we treat— this has simply been a sampling of studies that have been conducted to determine the efficacy of chiropractic. Chiropractic Treatment of Sciatica

Doctors of Chiropractic regularly treat sciatica. Sciatica is characterized by pain that originates in the low back or buttock that travels into one or both legs. Sciatic nerve pain varies in intensity and frequency; minimal, moderate, severe and occasional, intermittent, frequent or constant.

Pain is described as dull, achy, sharp, toothache-like, pins and needles or similar to electric shocks. Other symptoms associated with sciatica include burning, numbness and tingling sensations. Sciatica is also called radiating or referred pain, neuropathy, or neuralgia. A misconception is that sciatica is a disorder - however, sciatica is really a symptom of a disorder.

Caused by Nerve Compression
Sciatica is generally caused by irritation to the nerve roots that contribute to the sciatic nerve. Disorders known to cause sciatic nerve pain include lumbar spine subluxations (misaligned vertebral body/ies), herniated or bulging discs (slipped discs), pregnancy and childbirth, tumors, and non-spinal disorders such as diabetes, constipation, or sitting on one's back pocket wallet.

One common cause of sciatica is Piriformis Syndrome. Piriformis syndrome is named after the piriformis muscle. The piriformis muscle is located in the lower part of the spine, connects to the thighbone, and assists in hip rotation. The sciatic nerve runs beneath the piriformis muscle. This muscle is susceptible to injury from a slip and fall, hip arthritis, or a difference in leg length. Such situations can cause cramping and spasm to develop in the piriformis muscle, thereby pinching the sciatic nerve and causing inflammation and pain.

Sciatic nerve compression may result in the loss of feeling (sensory loss), paralysis of a single limb or group of muscles (monoplegia), and insomnia.

Proper Diagnosis
Since there are many disorders that cause sciatica, the chiropractor's first step is to determine what is causing the patient's sciatica. Forming a diagnosis involves a thoughtful review the patient's medical history, and a physical and neurological examination. Diagnostic testing includes an x-ray, MRI, CT scan and/or electrodiagnostic tests (nerve conduction velocity, electromyography). These examinations and tests help to detect possible contraindications to spinal adjustments and other chiropractic therapies.

Treatment Approach
The purpose of chiropractic treatment is to help the body's potential to heal itself. It is based on the scientific principle that restricted spinal movement leads to pain and reduced function and performance. Chiropractic care is non-invasive (non-surgical) and drug-free. The type of chiropractic therapy provided depends on the cause of the patient's sciatica. A sciatica treatment plan may include several different treatments such as ice/cold therapies, ultrasound, TENS, and MOST IMPORTANTLY - spinal adjustments.

1. Ice/Cold therapy reduces inflammation and helps to control sciatic pain.

2. Ultrasound is gentle heat created by sound waves that penetrates deep into tissues. Ultrasound increases circulation and helps to reduce muscle spasms, cramping, swelling, stiffness, and pain.

3. TENS unit (transcutaneous electrical nerve stimulation) is a small box-like, battery-powered, portable muscle stimulating machine. Variable intensities of electrical current control acute pain and reduce muscle spasms. Larger versions of the home-use TENS units are used by chiropractors, physical therapists and other rehab professionals.

4. Adjustments - At the core of chiropractic care are spinal adjustments. Adjustments free restricted movement of the spine and helps to restore misaligned vertebral bodies to their proper position in the spinal column. Spinal adjustment helps to reduce nerve irritability responsible for causing inflammation, muscle spasm, pain, and other symptoms related to sciatica. Adjustments should not be painful. Spinal adjustments are proven to be safe and effective.

In college and during their training, students of chiropractic learn many different adjustment techniques enabling them to treat several types of subluxations and disorders. Techniques vary from a swift high velocity thrust to those that combine minimal force and gentle pressure. Mastery of each technique is an art that requires great skill and precision. Spinal manipulation is the treatment that differentiates chiropractic care from other medical disciplines.

To prepare a patient for adjustment, the chiropractor may instruct the patient to sit up or lie down. It is not uncommon for the chiropractor to use a totally different adjustment technique during the patient's subsequent visit. Treatment tables differ too. Some are stationary, flat padded tables and others are elaborate with electrically or manually operated head and foot rests.

Chiropractic's Limitations
Sciatica can be caused by other disorders beyond the scope of chiropractic practice. If the Doctor of Chiropractic determines the patient's disorder requires treatment by another type of doctor, then the patient is referred to another specialty. In some cases, the referring chiropractor may continue to treat the patient and co-manage the patient's care with the specialist.
